The Human-Centered Health Home Model

This episode features a conversation with proponents of the “human-centered” health home model, or “patient-centered” medical homes. What exactly is a human-centered health home? Dr. Pat Jonas and his colleague Steven Deal join us and explain that it is an innovative way to provide more comprehensive medical care to patients.

Ultimately, the human-centered health home is a concept, rather than a physical structure. Within that concept, a patient is provided a personal team of physicians, who focus on addressing all of the patient’s needs.

The care is coordinated and integrated and also addresses quality/safety issues, evidenced-based clinical decision making, enhanced access for patients, better communication between physicians and the patient, and payment issues for both the physicians and the patients. Download this episode to learn more!

Dr. Jonas is a past President of the Ohio Academy of Family Physicians, and current owner and president of Family Health Connections, Inc., a family focused family medicine practice where he works as a family physician.  He has also been the President of the Center for Innovation in Family and Community Health for more than 11 years. Dr. Jonas hosts his own radio show, entitled “Dr. Synonymous,” which is featured weekly along with his blog by the same title at drsynonymous.blogspot.com.

Steven Deal, an MIT graduate in astronautics and aeronautics, serves as the secretary and a volunteer board member for the Center for Innovation in Family and Community Health.
